Job Title: Pharmacy Ambulatory Care

The clinical pharmacy specialist will provide direct patient care under collaborative practice agreements as part of an interdisciplinary team. Ambulatory pharmacists provide care in hospital-based outpatient clinics and as an embedded resource in physician-based clinics throughout the Physician Group, with a focus on anticoagulation management and diabetes management with cardiovascular risk reduction. The pharmacists manage anticoagulation in hospital-based outpatient clinics via point of care office visits, enoxaparin bridging, patient self-testing, and evaluate and counsel patients on direct oral anticoagulants.
Position is Monday-Friday Days
PGY1 – preferred
2 years experience in Ambulatory Care
